個人情報の取扱に関する同意書
株式会社LifeArk（以下、「当社」といいます。）は、求職者（以下、「本人」といいます。）から提供された個人情報を、以下の通り適切に取り扱うことをお約束します。
1. 収集する個人情報
当社は、以下の個人情報を収集します。
・氏名
・住所
・電話番号
・メールアドレス
・生年月日
・その他、求職活動に必要な情報
2. 利用目的
収集した個人情報は、以下の目的にのみ使用いたします。
・求職者の希望に沿った職場の紹介
・当社の提携会社（採用を行う企業や他の職業紹介事業者等）への開示
・求職者への連絡や業務上必要な対応
3. 個人情報の第三者提供
当社は、上記利用目的の範囲内で、本人の同意のもと、提携会社に対して個人情報を提供いたします。提携会社は、求人や採用に関する目的のために個人情報を使用し、他の目的では使用いたしません。その他、法令に基づく場合を除き、本人の同意なく第三者に個人情報を提供することはありません。
4. 安全管理のために講じている措置
当社は、個人情報の漏えい、滅失及び毀損などのリスクに対して、個人情報の安全管理が図られるよう、必要かつ適切な措置を講じているほか、当社の従業員に対し、必要かつ適切な監督を行っています。なお、安全管理措置の内容の詳細をお知りになりたい場合は、以下の「連絡先」までご連絡ください。遅滞なく回答いたします。
5. 個人情報の委託
前述の利用目的の範囲内で、個人情報の取扱いを委託することがあります。委託にあたっては、委託先における個人情報の安全管理が図られるよう、委託先に対する必要かつ適切な監督を行います。
6. 同意の撤回
本人は、個人情報の提供について同意を撤回することができます。撤回を希望される場合、当社の指定する窓口にご連絡ください。
7. 個人情報の開示・訂正・削除について
本人は、当社に対して自身の個人情報の開示、訂正、削除を請求することができます。請求に際しては、当社の個人情報保護担当者にご連絡ください。
